The Omaha System Community of Practice
is a collaborative workgroup of public health and home care agencies, educators, and software vendors who use the Omaha System. OSCOP is a place to share Omaha System resources.
It is also an active workgroup with the purpose of improving and standardizing public health practice, documentation, data collection, and outcome reporting.
The OSCOP website is a central location to share our work with each other and with the wider Omaha System community. Some of the things you will find here include:
- Omaha System careplans from diverse programs and agencies,
- KBS rating guide supplements,
- examples of ways to present Omaha System data, and
- academic projects and publications.
We welcome your use of the information with your acknowledgment, and appreciate your feedback.
PH-Doc Offers CCD/C-CDA Document
PH-Doc (Public Health Documentation System) has been sending and receiving Continuity of Care documents with exchange partners since 2012. The outgoing document includes a summary of services provided (using the Encounters Section), medications 'reported', allergies 'reported', diagnoses, Omaha System problems, vitals, smoking status, immunizations, along with Omaha System signs and symptoms and KBS ratings (using the Results Section). Agencies control how many years/months of history is included. Incoming documents can be reconciled and imported into the client/patient chart.

Nightingale Notes Offers Continuity of Care Document
Nightingale Notes offers a Continuity of Care Document (CCD) to support the national goal of data exchange at patient transitions of care. This report has emerged as a consensus priority from the national Meaningful Use mandate. The CCD includes data on demographics, medications, problems, conditions, results, and social history. This data is coded using national standards, and the outline format is standardized to make it easy for a health care provider to find information. The problems section contains Omaha System problems with mapping to SNOMED CT, and the results section includes Omaha System signs/symptoms and KBS ratings. Available in PDF, HTML, XML, and HL7; this human-readable document can also be read computers and incorporated within other software systems.

2010 KBS Rating Guide Supplement Completed
This version of the Omaha System KBS Rating Guide Supplement was updated in 2010 by over 200 international Omaha System users. Our goal is to support inter-rater reliability of Omaha System KBS rating data for research and program evaluation around the globe. We will be reviewing and updating the guide again in 2011. If you have suggestions for changes, please participate in the next review (details will be posted on this web page).

The Omaha System Partnership for Knowledge Discovery and Health Care Quality
The Omaha System Partnership for Knowledge Discovery and Health Care Quality is a new initiative within the University of Minnesota School of Nursing.
The partnership has 3 components:
- a multidisciplinary scientific team of University of Minnesota researchers with experience in advanced data analysis and data mining techniques
- a group of affiliate members from 5 countries who contribute clinical Omaha System data, suggest important clinical questions, and work together with the scientific team on research and evaluation projects
- a warehouse of de-identified clinical Omaha System data including client problems, signs/symptoms, interventions, and knowledge, behavior, and status outcomes
